Ny profesjon i legevakt?

Abstract

Bakgrunn: Norges demografiske utvikling sammen med samhandlingsreformen og fastlegemangel har bidratt til at legevakt opplever tidvis stor pasientpågang og lange ventetider, spesielt for pasienter med lav hastegrad. Innføring av avansert klinisk sykepleie i legevakt vil trolig føre til bedre oppgaveglidning, mer effektiv helsetjeneste og kortere ventetid.

Hensikt: Hensikten med denne studien er å utforske og beskrive legenes erfaringer med bruk av avansert klinisk sykepleie i legevakt.

Metode: Studien benytter kvalitativ metode, og data ble innhentet med seminstrukturerte dybdeintervjuer av en gruppe utvalgte leger som jobber i legevakt.

Resultat: Resultatene av analysen viste at legenes erfaringer kunne deles inn fem hovedkategorier: Fordeler, utfordringer, arbeidsoppgaver, samarbeid og kunnskaper.

Konklusjon: Legene var positive til bruk av AKS i legevakt. De så på AKS som en ressurs de kunne benytte seg av, men også i en selvstendig behandlerroller til enkelte pasienter og problemstillinger. Utfordringer var blant annet mangel på kunnskaper om AKS hos legene, og utilfredsstillende avklaring av rolle- og ansvarsfordeling.

Background: Norway's demographic development, together with the collaboration reform and the shortage of general practitioners, has contributed to the out-of-hours primary clinic experiencing occasional large patient demand and long waiting times, especially for patients with a low degree of urgency. The introduction of advanced clinical nursing in the out-of-hours primary clinic will probably lead to better task slippage, more efficient health services and shorter waiting times.

Purpose: The purpose of this study is to explore and describe the doctors' experiences with the use of nurse practitioner in the out-of-hours primary clinic

Method: The study uses a qualitative method, and data were obtained with semi-structured in-depth interviews of a group of selected doctors who work in the out-of-hours primary clinic

Results: The results of the analysis showed that the doctors' experiences could be divided into five main categories: Benefits, challenges, work tasks, collaboration and knowledge

Conclution: The doctors were positive about the use of nurse practitioner in the out-of-hours primary clinic. They found nurse practitioners as a resource they could use, but also in independent treatment roles for some patients and issues. Challenges included a lack of knowledge about nurse practitioners among the doctors, and unsatisfactory clarification of the division of roles and responsibilities.
