Hvilke erfaringer har sykepleierne med bruk av videokonferanse i legevaktsentral?

Abstract

Bakgrunn:

Legevaktsentralene i Norge opplever et økende antall henvendelser pr telefon og tidvis stor pasientpågang. Det medfører høyere krav til selektering av telefonhenvendelsene for å redusere antall fysiske oppmøter i legevakt. Videokonferanse er tatt i bruk ved nesten 50% av landets legevaktsentraler, men det eksisterer lite kunnskap som gjelder sykepleiernes erfaringer med bruk av videokonferanse.

Hensikt:

Utforske og beskrive erfaringer og utfordringer sykepleierne har med bruk av videokonferanse.

Metode:

Det er benyttet kvalitativ metode i studien. Fire fokusgrupper med til sammen 16 sykepleiere ble intervjuet og dannet datagrunnlaget for studien. Lydbåndopptakene ble transkribert og analysen er inspirert av Tematisk analyse.

Resultater:

Analysen avdekket tre overordnede temaer for erfaringene sykepleierne hadde med videokonsultasjon; erfaringer, kommunikasjon og utfordringer. Informantene hevdet videokonferanse bidro til bedre og mer hensiktsmessig kommunikasjon med innringer, spesielt ved vurdering av helsetilstand og hastegrad. Videokonferanse ble oppfattet ressursbesparende bl.a. med tanke på tidsbruk, økonomi og bruk av personell. Utfordringer var sykepleiernes kliniske kompetanse, tekniske vansker, dokumentasjon og etikk.

Konklusjon

Studien viser at informantene var positive til bruk av videokonferanse. Flere av sykepleierne oppga å bruke videokonferanse for vurdering av almenntilstand, spesielt for små barn og eldre. Bruk av videokonferanse for verifisering av egne vurderinger, spesielt ved grønn hastegrad, ble også framhevet av informantene. Sykepleierne opplevde iten grad av utfordringer med videokonferanse.

Background:

Emergency centers in Norway are experiencing an increasing number of calls by phone and sometimes a large number of patients. This entails higher requirements for the selection of telephone inquiries in order to reduce the number of physical attendances in the emergency department. Video conferencing has been adopted at almost 50% of the country's emergency centres, but there is little knowledge regarding the nurses' experiences with the use of video conferencing.

Purpose:

To explore and describe experiences and challenges the nurses have with the use of videoconferencing.

Method:

A qualitative method has been used in the study. Four focus groups with a total of 16 nurses were interviewed and formed the basis for the study. The audio recordings were transcribed, and the analysis is inspired by Thematic analysis.

Results:

The analysis revealed three overarching themes; experiences, communication and challenges. The informants claimed that video conferencing contributed to better and more appropriate communication with callers, especially when assessing the state of health and degree of urgency. Video conferencing was perceived as saving resources in terms of, among other things, time consumption, finances and the use of personnel.

Conclusion:

The study shows that the informants were all positive about using videoconferencing. Many noted the need to have good clinical skills. Several of the informants stated that they used videoconferencing to assess the general condition, especially for young children and the elderly. The use of video conferencing for verification of own assessments, generally in the case of low urgency, was also highlighted. The nurses experienced å small degree of challenges with videoconferensing.
