Kommunale helse-og omsorgstjenester til voksne personer med utviklingshemming og psykisk lidelse. - En kvalitativ undersøkelse av fire fagpersoners erfaringer fra bofellesskap.

Abstract

Bakgrunn: Kommunale helse-og omsorgstjenester til personer med utviklingshemming og psykisk lidelse er lite studert. Landsomfattende tilsyn med tjenestene til personer med utviklingshemming avdekker til dels svært alvorlige forhold. Tjenesteyterne har svak kompetanse og mange ansatte er dårlig informert om brukerne de bistår. Kunnskapsgrunnlaget på daglig oppfølging og behandling i bofellesskap er tynt.

Hensikt: Formål med studien er å undersøke om det foregår behandling av psykisk lidelse hos personer med utviklingshemming i bofellesskap, samt innsikt i erfaringer personal har fra samhandling innad i bofellesskapet og med andre instanser.

Metode: Studien har et kvalitativt design, og baserer seg på semi-strukturerte intervju med fire fagpersoner ansatt i bofellesskap i ulike kommuner. Tematisk tekstanalyse ble benyttet som metode for analyse av data.

Funn: Personal i bofellesskap utfører miljøbehandling med fokus på relasjoner, lavaffektiv tilnærming, og støtter bruker i å strukturere hverdagen. Hinder for å utføre miljøbehandling kan være; utfordringer med kompetanse i personalgruppen, lite tid til samhandling rundt brukerne, brukerne har for mange personer å forholde seg til. Samhandling med fastlege og habiliteringstjenesten for voksne byr på utfordringer. Personalet erfarer at fastlegen ikke alltid tar brukernes symptomer på alvor. Personale i bofellesskap er skeptiske til kompetansen i habiliteringstjenesten.

Konklusjon: Personalet i bofellesskap utfører miljøbehandling. Dagsplan er et viktig redskap. Elementer fra lavaffektiv samhandling likeså.

For store personalgrupper kan hindre gjennomføring av miljøbehandling, planlegging, evaluering og samhandling i personalgruppen.

Background: Residential healthcare services to persons with ID and mental illness is an understudied topic.

A survey conducted by the Norwegian Directory of Health uncovered unsatisfactory situations. Service providers lack required skills and have little knowledge about the persons they assist. Research on day to day care in residential living is scarce.

Aim: The aim of the study is to explore treatment of mental illness in persons with ID living in residential care facilities (Residents). Does such treatment occur? What are the caregiver’s experiences on interaction within the workplace and also with specialist services?

Methods: This is a qualitative study. Four caregivers from different municipalities participated in semi structured interviews. Thematic text- analysis was used to analyse the data.

Findings: Caregivers in residential living facilities work within milieu therapeutic practice. They focus on personal relations and low expressed emotion. They support the residents in structuring their lives. Hindrances for good milieu therapeutic practise can be; lack of professional skills, too little time to interact with, and to discuss with colleagues, too many persons working with each resident. Joint interaction with primary physician and specialist services for persons with ID is challenging. Specialist services for persons with ID may not have sufficient knowledge on mental illness.

Conclusion: Residential caregivers perform milieu therapy. Daily structure for the residents is an important element, as is low expressed emotion. Too many caregivers working with each resident can be a hindrance when it comes to planning, evaluating, staff interaction and thereby performing good milieu therapy.
