dc.description.abstractINNLEDNING: Escherichia coli er en del av den fekale normalfloraen hos mennesker og varmblodige pattedyr, men kan også forårsake ekstraintestinale infeksjoner. Særlig kjent er den som etiologisk agens ved ukompliserte urinveisinfeksjoner. Resistensutvikling mot antibakterielle midler som brukes i behandlingen av UVI er et kjent problem, både på verdensbasis og i Norge. En av måtene resistens kan spres på er via plasmider. Man har undersøkt plasmidmediert overførbar resistens i to ulike materialer. Målet med oppgaven er å se hva overførbare plasmider har å si for resistensprofilen hos isolatene i de to ulike materialinnsamlingene. METODER: Det ble utført konjugasjonsforsøk (kolonikryssning på fast agar) med 138 fekale E. coli (tilhørende 20 kloner) fra friske kvinner og 35 ciprofloxacinresistente uropatogene E. coli (tilhørende 25 kloner) som donorer. E. coli DH5α pUC19 NalR eller E. coli J53 AzR ble benyttet som primære mottakere i konjugasjonsforsøkene og E. coli HB101 StrR som sekundær mottaker. Ved hjelp av agardiffusjonstest og E-test ble transipientene fra det fekale normalmaterialet undersøkt for resistens mot sulfonamid, trimetoprim eller tetracyclin, mens transipientene fra det ciprofloxacinresistente UVI-materialet ble undersøkt for resistens mot ampicillin, sulfonamid, trimetoprim, tetracyclin, ciprofloxacin, nalidixinsyre, kloramfenikol eller kanamycin. Transipientenes og donorenes innhold av plasmider ble undersøkt med restriksjonskutting, kutting med S1-nuklease og S1-PFGE. RESULTATER: Åtte av 138 isolater i det fekale normalmaterialet hadde overførbar resistens mot både sulfonamid og trimetoprim. Ingen overførte resistens mot tetracyclin. Alle isolatene med overførbar resistens tilhørte samme klon med likt resistensmønster. Generelt var flere plasmider overført fra donor til de primære transipientene selektert med sulfonamid, enn til de primære transipientene selektert med trimetoprim. I sekundære konjugasjonsforsøk fikk en kun overført resistens videre fra de primære sulfonamidtransipientene, ikke de primære trimetoprimtransipientene. De sekundære sulfonamidtransipientene var likevel resistente mot både sulfonamid og trimetoprim. Resistensdeterminanten(e) satt mest sannsynlig på et ikke-konjugerbart plasmid med størrelse på ca 7,8 kb (gitt navnet pCTL5). De(t) konjugerbare plasmidet/ene er ikke bestemt, men er mest sannsynlig et plasmid på ca 35 kb (gitt navnet pCTL3). Av 35 isolater i det ciprofloxacinresistente materialet hadde 13 stk, tilhørende 11 kloner, overførbar resistens mot ett eller flere antibiotika. Totalt fikk en vekst av 21 sikre transipienter, samt tre usikre. Resistens mot nalidixinsyre og/eller ciprofloxacin lot seg kun overføre ved indirekte seleksjon, mens resistens mot ampicillin, sulfonamid, trimetoprim, tetracyclin, kloramfenikol eller kanamycin lot seg overføre både ved direkte og indirekte seleksjon. Bruk av kanamycin som seleksjonsmiddel ga hyppigst vekst av transipienter, mens det var resistens mot ampicillin og/eller sulfonamid som var hyppigst(e) overført(e) resistendeterminant(er). The bacteria may also be the cause of extra-intestinal infections, i.e. nearly 80 % of uncomplicated urinary tract infections (UTIs). Development of resistance against antibiotics used in treatment of UTI is a well known and escalating problem worldwide. Amongst others, the resistance genes can spread between bacteria on plasmids. One has studied plasmid-mediated antibiotic resistance in two materials; 1) Faecal E. coli from healthy women, and 2) ciprofloxacin resistant urinary E. coli from out-patients diagnosed with uncomplicated UTIs. METHODS: Conjugation experiments (colony cross streak) were performed using 138 faecal E. coli (belonging to 20 clones) from healthy women and 35 urinary ciprofloxacin resistant E. coli (belonging to 25 clones) as donors. E. coli DH5α pUC19 NalR or E. coli J53 AzR were used as receivers in the primary conjugation experiments, while E. coli HB101 StrR were used as a secondary receiver. Using disk diffusion tests and E-tests, the transcipients from experiments with faecal E. coli were tested for antibiotic resistance against sulphonamide, trimethoprim and tetracycline while the transcipients from experiments with ciprofloxacin resistant E. coli were tested for resistance against ampicillin, sulphonamide, trimethoprim, tetracycline, ciprofloxacin, nalidixic acid, chloramphenicol or kanamycin. Plasmid investigations were carried out using S1-PFGE, S1-nuclease cutting and restriction analysis. RESULTS: Eight out of 138 faecal E. coli possessed transferable antibiotic resistance against both sulphonamide and trimethoprim. None were able to transfer resistance against tetracycline. The eight isolates were all members of the same clone and shared equal resistance pattern. Generally, more plasmids were transferred to the primary transcipients when using sulphonamide as a selection agent than when using trimethprim. In the secondary conjugation experiments, the primary sulphonamide transcipients were able to further transfer resistance while the primary trimethoprim transcipients were not. Still the secondary transcipients were resistant against both sulphonamide and trimethoprim. The resistance determinants can most likely be found at a non-conjugative plasmid sized ap. 7,8 kb (given the name pCTL5). The conjugative plasmid has not been defined, but is assumed to be a plasmid given the name pCTL3 (ap. 35 kb). Out of 35 urinary ciprofloxacin resistant E. coli isolates, thirteen isolates belonging to 11 clones possessed transferable antibiotic resistance against one or more antibiotics. Totally 21 certain, and three uncertain, transcipients were obtained. All resistance determinants were able to transfer using both direct and indirect selection, except against quinolones where only indirect selection gave growth of transcipients. Using kanamycin as selection agent gave the most frequent growth of transcipients, while it was resistance against ampicillin and/or sulphonamide that were the most frequent resistance determinants to be transferred. Plasmids found in the transcipients varied in size from ap. 40 to ap. 240 kb.
