dc.description.abstractBakgrunn/Hensikt: Skrøpelige eldre preges av multimorbiditet og komplekse helseproblemer, og blir mer sårbare for sykdom på grunn av kroppens svekkede evne til homøostase. Comprehensive geriatric assessment er en diagnostisk behandlingsprosess utviklet for å imøtekomme denne gruppens komplekse og omfattende behov. Jeg ønsket å finne ut hvilke styrker og svakheter comprehensive geriatric assessment kan ha for skrøpelige eldre som får en akutt endring i sin helsetilstand, og hvilken rolle denne prosessen kan ha i primærhelsetjenesten. Metode: Jeg har valgt å gjøre en litteraturstudie. 10 forskningsartikler ble inkludert i studien som ble sammenstilt og systematisert i tema inspirert av tematisk analyse. Resultater: Mine funn viser at comprehensive geriatric assessment til skrøpelige eldre som opplever en akutt endring i sin helsetilstand har flere styrker. Comprehensive geriatric assessment kan føre til at sannsynligheten for å endre omsorgsnivå synker. Funn viser også at skrøpelige pasienter bedre bevarer og forbedrer funksjonsnivå og får mindre progresjon av sin skrøpelighet. Dette uten at totalkostnadene øker. Comprehensive geriatric assessment har ifølge mine funn ingen åpenbare svakheter, men på noen områder viser funnene lite, varierende eller ingen endring for de skrøpelige pasientene til tross for denne prosessen. Konklusjon: Comprehensive geriatric assessment har mange styrker som kan føre til gode resultater på viktige områder både for den enkelte og samfunnet, og bør implementeres og standardiseres med sjekklister og validerte vurderingsverktøy. Det bør etableres helhetlige pasientforløp som inkluderer comprehensive geriatric assessment for skrøpelige eldre som legges inn på kommunal akutt døgnenhet.
dc.description.abstractBackground: Frail elderly are characterized by multimorbidity and complex health problems, and become vulnerable to disease due to their bodys`s impaired ability to homeostasis. Comprehensive geriatric assessment is a diagnostic treatment process developed to meet the complex and comprehensive needs of this group. I wanted to find the strengths and weaknesses comprehensive geriatric assessment can have to frail elderly who experience an acute change in their state of health, and if this process should be used in the primary health care. Methods: I have chosen to do a litterature review. Ten research articles were included in this study, whitch was systematized in themes inspired by thematic analysis. Results: My findings show that comprehensive geriatric assessment for frail elderly people who experience an acute change in their state of health has several strengths. Comprehensive geriatric assessment decreases the likelyhood to be discharged to an increased level of care. Findings also show that frail patients better preserve their level of function and have less progression of their frailty, at no higher cost. According to my findings, comprehensive geriatric assessment has no obvios weaknesses, but in some areas the findings show little, varying or no differences between the groups despite this process. Conclusions: Comprehensive geriatric assessment has many strengths that can lead to good results in important areas both for the individual and the society, and should be implemeted and standardized with checklists and validated assessment tools. A comprehensive patient course should be developed that includes comprehensive geriatric assessment for frail elderly people who are admitted to a muncipal emergency unit.
