Samarbeid mellom hjemmesykepleie og legevaktbil

Abstract

Bakgrunn: Kommunehelsetjenestens utfordringer med stadig økende antall av hjemmeboende multisyke pasienter utfordrer kommunens plikt til å tilby forsvarlige helsetjenester. Legevaktbil er en tjeneste for kommunens innbyggere som tilbyr medisinsk vurdering av ulike sykdomstilstander i pasientens eget hjem utenom fastlegers kontortid. Legevaktbil er dermed en viktig samarbeidspartner for hjemmesykepleien som er nærmest pasienten og er ansvarlig for kartlegging, oppfølging og iverksetting av tiltak hos pasienter med sammensatte lidelser. Det finnes midlertidig lite kunnskap om hvordan en slik ambulerende helsetjeneste fungerer og hvordan sykepleiere i hjemmesykepleien erfarer samarbeid med legevaktbil.

Hensikt: Hensikten med denne studien er å utforske og beskrive sykepleieres erfaringer av samarbeid med legevaktbil hos hjemmeboende pasienter.

Metode: Studien hadde kvalitativ design. Datainnsamlingsmetode bestod av ti semistrukturerte intervjuer av sykepleiere som jobbet i hjemmesykepleien. Analysen var inspirert av tematisk analyse.

Resultat: Gjennom analysen kom det frem to hovedkategorier, fremmende og hemmende faktorer for samarbeid. Hovedtyngden av de fremmende faktorene for samarbeid bestod av god kommunikasjon og deltagernes inkludering i pasientvurdering. De hemmende faktorene var preget av manglende kommunikasjon og uklar ansvarsfordeling mellom legevaktbil og hjemmesykepleie.

Konklusjon: Deltagerne hadde ulike erfaringer knyttet til samarbeid med legevaktbil. Den mest fremmende faktoren så ut til å være god kommunikasjon med legevaktbil i hele kommunikasjonskjeden. Det var flere faktorer som var hemmende for samarbeid. Manglende kommunikasjon både før-, under- og etter hjemmebesøk av legevaktbil var fremtredende.

Background: The municipal health service's challenges with the ever-increasing number of multi-illness patients living at home, challenge the municipality's duty to offer sufficient health services. An ambulant physician-paramedic-team is a service for the municipality's residents that offers medical assessment of various medical conditions in the patient's own home outside the GP's office hours. This ambulant service is thus an important partner for home health care, where nurses are closest to the patient and is responsible for identifying symptoms, follow-up and implementation of measures in patients with complex disorders. There is temporarily little knowledge about how such an ambulatory service works and how nurses in home health care experience cooperation with this service.

Purpose: The purpose of this study is to explore and describe nurses' experiences of collaboration with ambulant physician-paramedic-team in patients living at home.

Methods: The study had a qualitative design. The data collection method consisted of ten semi-structured interviews with nurses who worked in home health care. The analysis was inspired by thematic analysis.

Results: Through the analysis, two main categories emerged, promoting and inhibiting factors for collaboration. The majority of the promoting factors for collaboration consisted of good communication and the participants' inclusion in patient assessment. The inhibiting factors were characterized by a lack of communication and an unclear division of responsibilities between the physician-paramedic-team and nurses in home health care.

Conclusion: The participants had different experiences related to cooperation with the ambulant physician-paramedic-team. The most promoting factor seemed to be good communication with the physician-paramedic-team throughout the entire communication chain. There were several factors that inhibited cooperation. A lack of communication both before, during and after home visits by the team was prominent.
