Hvordan beskriver anestesisykepleiere sin kompetanse når de ivaretar pasienter med massive blødninger peroperativt: en kvalitativ studie

Abstract

Bakgrunn: Dette krever at anestesisykepleieren har en tydelig forståelse av sitt funksjons- og ansvarsområde, og at anestesisykepleieren har evnen til å kritisk analysere sin egen kompetanse.

Hensikt og problemstilling: Å undersøke hvordan anestesisykepleiere beskriver sin kompetanse når de ivaretar pasienter med massive blødning peroperativt.

Metode: Studien har et kvalitativt design med en eksplorativ tilnærming hvor fokusgruppeintervju anvendes for å samle inn anestesisykepleiernes erfaringer ved bruk av en semistrukturert intervjuguide. Data ble analysert ved hjelp av Graneheim og Lundman (2004) sin kvalitative innholdsanalyse.

Funn: Analyseprosessen resulterte i et tema: Ikke-tekniske ferdigheter er en sentral del av anestesisykepleieren sin kompetanse, to hovedkategorier: Forberedt på det uforutsigbare og Godt teamarbeid er avgjørende og fem underkategorier: Ansvar i rollen, Være i forkant, Årvåkenhet og faglige vurderinger, Samspill og samarbeid og Manglende kontroll.

Konklusjon: Studien konkluderer med at ikke-tekniske ferdigheter er avgjørende i møte med massivt blødende pasienter peroperativt. Evnen til å samarbeide i team og anestesisykepleieres vurderings- og beslutningskompetanse ble trukket frem som sentrale elementer i anestesisykepleieres profesjonelle kompetanse. Det er fremdeles mye å lære om anestesisykepleierens kompetanse i møte med massive blødninger, men studiens funn tyder på at ikke-tekniske ferdigheter bør integreres allerede under utdanningen av anestesisykepleiere fordi vurderings- og beslutningskompetanse utvikler seg gjennom erfaring.

Background: Massive perioperative bleeding poses a risk to patient safety. As a member of the surgical team, the nurse anesthetist has an active part in ensuring that the team works together to solve the challenges massive bleeding represents. This requires a clear understanding of the nurse anesthetist’s role and area of responsibility. The nurse anaesthetist must also have the ability to critically analyze their own competence.

Purpose and research question: To explore how nurse anesthetists describe their competence when caring for patients with massive bleeding perioperatively.

Method: The study has a qualitative design with an exploratory approach. Two focus groups were held to gather the nurse anesthetists’ experiences using a semi-structured interview guide. The data was analyzed using Graneheim and Lundman’s method of content analysis.

Findings: The analysis process resulted in one theme: Non-technical skills are a central part of a nurse anesthetist’s competence, to main categories: Being prepared for the unpredictable and Good teamwork is crucial, and five subcategories: Responsibility in the role, Thinking ahead, Vigilance and professional assessments, Interaction and cooperation and Lack of control.

Conclusion: The study concludes that non-technical skills are crucial in dealing with massively bleeding patients. The ability to collaborate in teams, and the nurse anesthetist´s assessment and decision-making competence were highlighted as key elements in the nurse anesthetist´s professional competence. There is however still much to learn about the nurse anesthetist’s competence in the face of massive perioperative bleeding, but the findings in this study suggest that non-technical skills should be integrated as part of the nurse anesthetist´s education, since assessment and decision-making skills develop through experience.
